PROJECT RISK MANAGEMENT & PROCESS IMPROVEMENT

23 February, 2015

(The explanation in this short article originated from many sources especially Wikipedia. They have been rewritten to suit the respective clients' requirements. The explanation do not represent the actual contents of the notes/materials used for briefing, training and consultancy)

Project risk management include; not limited to the following; 5 significant elements :

- Plan and manage the risk,

- Identify the risk
- Qualitative and quantitative risk analysis
- Planning risk responses
- Monitor and control

Risk management is one of the ten knowledge areas in which a project manager must be competent

Project risk is defined by PMI as 'an uncertain event or condition that, if it occurs, has a positive or negative effect on a project’s objectives

Good Project Risk Management depends on supporting organisational factors, clear roles and responsibilities, and technical analysis skills

Process improvement in any organization must be systematic in order to optimize its underlying processes to achieve more efficient results.

Leaders are responsible for creating the strategic action plans in order to drive the organization towards success

 PLAN-DO-CHECK-ACT

 Start Small - End Big

-Set clear timelines.

- Do not spread resources thinly and focus on the short term payoff.
-Management and primary stakeholders must be involved, or else even a limited implementation will fail.

It is the methodology that both Process Redesign and Business Process Reengineering are based upon.

FATE MADE US MEET (NIK ZAFRI MEETS YAB (Allahyarham) NIK AZIZ)

(Note : The highlight is done purposely)
Against all odds, I was the only civillian participating in the Law Journal Competition - National Level (certificate signed by the then and Allahyarham Dean of IIU (the legendary Tan Sri Datuk Prof. Ahmad Mohamed Ibrahim) and the then/Allahyarham Hon. Minister of Justice.

The theme of my journal was 'Syariah Law and Civil Law (and its' role in the Constitution)'

I sent a copy to a friend of mine, (now Dato' Paduka) Haji Husam Musa (was then the Press Secretary to Hon. Chief Minister/Menteri Besar of Kelantan) to be forwarded to (Allahyarham) YAB Tuan Guru (Dato' Bentara Setia Haji) Nik Abdul Aziz Bin Nik Mat to get his comments (Allahyarham Tuan Guru was then - only one week holding the Honourable Chief Minister of Kelantan's post) - (a copy was also sent to Tun Dr. Mahathir Mohamed who was then the Prime Minister of Malaysia)

A week later, Hj. Husam faxed an invitation to my parents' office in Kota Bharu - to my surprise - I was granted an exclusive research interview (which lasted around 2 1/2 hours on the subject matter) with the YAB Menteri Besar of Kelantan himself.

I recalled when I first walked in, he greeted me with a 'Salam' and said "Kita duo ni anok branok sebenarnyo, jadi panggil 'Ayoh Zih' doh la deh" ("We are related, so just address me as 'Ayah Zih' (a common way to address an uncle)

It was a fruitful interview. He praised my efforts and expressed how proud he was seeing my hard work and even said that he foresee that I will become a successful person one day - - perhaps a bridge between the opposition and the government. ('Amin' to that)

A month after the interview, Haji Husam called up my parents saying that YAB Tuan Guru wanted to visit my parents in Lundang, Kota Bharu in his capacity as a relative. I recalled Hj Husam told my parents about the interview. (This was in 1990)
I had a surprise again. YAB Tuan Guru expressed gratitude to my parents including my aunt/uncle despite the political differences - for helping him out in the late 70s (nearly 20 years ago) when he was back from Mecca - was not feeling very well even having a high fever/vomitting/nausea etc. Tuan Guru ended up spending a night at my aunt/uncle's house in Petaling Jaya.
The best parts of (Allahyarham) YAB Tuan Guru Dato' Bentara Setia Haji Nik Abdul Aziz that I will always remember :
1. Despite the political differences, he would never sever the ties of relationship with relatives, (something that I did not see much today)
2. He remembers facts clearly (evident - during my research interview) and he never forgets what others have done good to him even 20 years have passed. Most people do not have that value in them anymore.
AL-FATIHAH.

TransAsia plane crashes in Taiwan river, killing at least 23 people (Source : The Guardian)

Dramatic footage shows regional airliner clipping taxi cab and bridge minutes after takeoff

Footage of TransAsia Airways plane crashing.

A TransAsia Airways flight in Taiwan carrying 58 passengers and crew careened past buildings, clipped a highway and crashed into a shallow stream, killing at least 23 people.

TransAsia GE 235, a domestic flight from Taipei to Kinmen – a small archipelago near mainland China – crashed at 10.56am local time, according to Taiwan’s aviation council, about three minutes after it took off. Astonishing dash-cam videos posted online showed the turboprop ATR 72-600 aircraft in its final airborne moments, turning vertical over a highway and clipping a taxi cab and a bridge with its left wing.

Rescuers are searching into the night for 20 missing people, after 15 were pulled alive from the wreckage.

“Several fire engines, ambulances, water craft and almost 170 rescue staff have been dispatched,” said a press release by the Taiwanese Central Disaster Response Centre. Local TV stations broadcasted footage of rescue workers in life vests and yellow helmets surrounding the plane’s fuselage in inflatable rafts.

At the moment, things don’t look too optimistic,” said Wu Jun-hong, a Taipei Fire Department official who was coordinating the rescue, according to the Associated Press. “Those in the front of the plane are likely to have lost their lives.”

The driver of the clipped taxi cab “has been sent to a local hospital”, an assistant to the Crown Taxi Company’s general manager who identified himself as Mr Yang told the Guardian.

“He has head injury and concussion, but all of his vital signs are stable.” Yang added that the company planned to raise the topic of compensation with TransAsia Airways at a later date.

The last communication from one of the aircraft’s pilots was “Mayday Mayday engine flameout”, according to an air traffic control recording on liveatc.net.

A flameout occurs when the fuel supply to the engine is interrupted or when there is faulty combustion, resulting in an engine failure.

The flight’s black box has been recovered, according to local media.

“Weather conditions were good and the pilot had 14,000 hours of flying hours and the co-pilot 4,000 hours,” Lin Zhiming, a representative from Taiwan’s Civil Aviation Authority, told reporters on Wednesday afternoon.

Among the passengers were 31 mainland Chinese tourists, travelling with two local travel agencies: Teyung Group, and Flying Tours.

Lin Liqing, manager of the Teyung Group, said that she had just arrived in Taipei to help with the handling of the incident.

“We are currently heading to the crash site and checking the passenger list with TransAsia Airways,” Lin said. She added that the passengers had been sent to eight local hospitals, and that she had not yet been able to visit them.

The manager of Flying Tours said that among 15 mainland Chinese tourists on the plane who were travelling with the agency, he had only confirmed one injured passenger – one of the two infants on board. He had no information on the remaining 14 people.

On Wednesday afternoon, the Taiwanese broadcaster TVBS showed rescuers pulling a toddler alive from the wreckage and rushing him or her to safety.

The chief executive of TransAsia, Chen Xinde, has publicly apologised for the crash.

Wednesday’s crash is the second by a TransAsia flight within the past six months — in July 2014, TransAsia flight ATR-72 crashed while attempting to land in the Penghu Islands soon after a typhoon, killing 48 people. The cause of the crash is still under investigation.

----------------------

Our deepest condolences to friends and family of the passengers and crew members including those who are affected by this sad tragedy.

Our hearts are with you.

Congratulations to the Rescue Team for the prompt action saving so many survivors and those who are hurt.
