HOW ALL CASE STUDIES AND ARTICLES ARE PRODUCED

Please note that the case studies and articles I share on topics relating to civil, structural, mechanical and electrical engineering, construction, economics, manufacturing, artificial intelligence, semiconductors, ESG, standards, codes of practice, and other related disciplines are not produced in isolation.

QHSEL Consultancy Sdn. Bhd. is supported by an advisory panel and an extensive network of trusted friends, industry practitioners, professional associations/bodies and subject matter experts who willingly contribute their knowledge, insights, and professional experiences.

This collective includes Professional Engineers with Practising Certificates (PEPC), licensed brokers, economists, academicians, competent persons, auditors, risk assessors, contractors, consultants, clients, enforcement officers, active and former intelligence operatives, IT and cybersecurity specialists, AI practitioners, VR/AR/MR professionals, architects, bankers, Syariah experts, and specialists from various other fields.

As such, this is far from a one-man effort. Behind every article is a panel of experienced professionals who provide guidance, technical input, peer review, and practical perspectives drawn from real-world experience.

Many of these contributors prefer to remain anonymous to protect the confidentiality of projects, organisations, and sensitive information entrusted to them. Their motivation is not recognition, but rather to contribute towards a greater purpose: promoting sound practices, strengthening governance, enhancing compliance, improving industry standards, and reducing deficiencies, non-conformances, and avoidable failures across various sectors.

The views presented are therefore often the result of multidisciplinary collaboration, professional discourse, and collective expertise rather than the opinion of a single individual.

IMPORTED CONSTRUCTION PRODUCTS: DOCUMENTATION IS PART OF ACCEPTANCE

When sourcing construction materials, equipment and systems say for e.g. from China, quality assurance begins well before shipment.

For equipment such as prefabricated pumping systems, valves and electrical switchboards, I or my appointed representative may need to visit the manufacturer to witness the Factory Acceptance Test (FAT), monitor the agreed inspections and obtain the supporting records. Depending on the scope of appointment, an independent testing or inspection body, such as TUV, may conduct or witness testing and issue the relevant reports or certificates. Once the FAT results and outstanding matters have been satisfactorily addressed, shipment can proceed, subject to the required approvals.

Upon arrival, we verify the delivery condition and supporting documentation. Site Acceptance Testing (SAT), where specified, follows installation to confirm performance under actual site conditions.

The acceptance criteria should be agreed before manufacture and reflected in both FAT and SAT procedures. These must address applicable Malaysian Standards (MS), specified British Standards (BS) or other accepted standards, project specifications and Malaysian regulatory requirements.

One practical challenge I have encountered is receiving technical documentation entirely in the manufacturer’s language. Test results, operating instructions and acceptance criteria require precise interpretation. Finding a translator who also understands the engineering terminology can take time, delaying review, installation and acceptance.

A constructive improvement is to specify the documentation language at procurement stage. Suppliers should provide complete documentation in Bahasa Malaysia and/or English, as agreed for the project, before shipment. Translations should be checked by technically competent personnel and supplied alongside the originals, with matching document numbers and revisions.

This requirement should cover test reports, certificates, drawings, manuals, safety instructions and maintenance information.

Clear documentation helps everyone involved from the manufacturer and consultant to the contractor, operator and approving authority make informed decisions.

Quality assurance includes ensuring that the evidence of compliance can be understood, verified and used throughout the asset’s life.

Sixteen With A Lifetime Still Ahead

Reading the reports about Runa, the 16-year-old girl in Japan who died following her detention, left me deeply saddened.

What particularly hurts to read is that she reportedly stopped eating during her detention and repeatedly expressed a desperate wish to see her mother. To imagine someone so young experiencing such distress is heartbreaking.

At sixteen, there should have been so much ahead of her: an education, friendships, opportunities, mistakes to learn from, and dreams she had perhaps not yet found the words to express.

As a father and grandfather, I find myself thinking about her mother. It is difficult to imagine the pain of losing a child, and the unanswered questions a parent must carry.

I respect the need for the facts and questions of responsibility to be examined fairly through the legal process. Even while those questions are being considered, there is room for compassion and reflection on how young people’s physical and emotional wellbeing can best be protected.

My heartfelt condolences to her mother and everyone who loved her. I hope they find answers, support and some measure of peace.

Sixteen is so young. She had so much life still to live.

THE GREAT CHASE OF KUALA LUMPUR (SOLARIS) - A Lovely Experience of Refined British Dining

A wonderful lunch at The Great Chase Kuala Lumpur - a lovely experience of refined British dining, generously treated by our second son, Nik Muhammad Hussainy, to celebrate our 37th wedding anniversary.

Our choices were the Charred Seabass, Angus Sirloin cooked medium rare, and Beef Sunday Roast.

The Angus Sirloin was the highlight for me. Chef Taufik delivered precisely the medium-rare finish I had hoped for, a standard I have encountered at only a handful of restaurants and hotels in Kuala Lumpur. Served with roasted tomatoes, fries and Cafe de Paris butter, it was a satisfying example of a classic steak dish prepared with care and confidence.

My wife was feeling a little under the weather, but she thoroughly enjoyed the Charred Seabass. Accompanied by mussels, a creamy tarragon sauce, smashed new potatoes with dill and caper puffs, the dish offered an inviting combination of seafood, gentle herbal notes and contrasting textures. A thoughtfully composed plate that made her lunch enjoyable despite not feeling her best.

Hussainy thoroughly enjoyed his Beef Sunday Roast, a generous celebration of traditional British comfort food. The beef comes with gravy, roasted potatoes, spiced carrots and cauliflower, sage-and-onion stuffing, neeps puree, greens and the essential Yorkshire pudding. With such a varied selection of accompaniments, it is a complete meal that captures the warmth and abundance associated with a proper British roast.

It was also a pleasure meeting Mr Izzat, the co-owner (I think), a fine young gentleman who is also involved in the construction industry, particularly mechanical and electrical works. Our conversation ranged across several subjects, and I was impressed by his knowledge, maturity and thoughtful observations. His manner speaks well of his upbringing and, in some ways, reminded me of my eldest son.

The staff were warm, friendly and attentive. They even prepared a special dessert for my wife and me to share, complete with a “Happy Anniversary” decoration, a thoughtful gesture that made the occasion all the more memorable.

One of these days, I will return to try their famous Beef Wellington. There is already a good reason for another visit!

Excellent food, gracious hospitality and precious time with family. A five-star experience in my book.

Thank you, Hussainy, and thank you to Chef Taufik, Mr Izzat and the team at The Great Chase for making our celebration so special.

PREVENTING FATAL FALLS: A HARNESS ALONE IS NOT ENOUGH

Falls are the most COMMON and DEADLIEST type of accident in construction.

A harness cannot compensate for inadequate clearance. In this example, 4.85 m is required but the lower floor is only 3.60 m below: a 1.25 m shortfall. Verify actual clearance against manufacturer instructions and site conditions.

ROOT CAUSES

Uncontrolled guardrail removal, poor planning, inadequate clearance checks, weak supervision and failure to reassess changed conditions.

PREVENTION

- Use HIRARC and a task-specific method statement to define the work sequence, edge protection, anchorage, clearance, access, responsibilities and rescue arrangements, and

Prioritise collective protection. Supervisors must brief workers, inspect controls and stop unsafe work. Review assessments whenever conditions change.

IF A FALL OCCURS

- Issue a stop-work order, isolate the area, activate the emergency plan and call 999.

- Mobilise trained rescuers and first-aiders. If unresponsive and not breathing normally, start CPR/use an AED as trained or directed. Avoid unnecessary movement unless essential for safety or lifesaving care. Obtain medical assessment.

- Do not assume death. Notify police for a suspected fatality, preserve evidence without delaying rescue, and protect casualty dignity.

- Notify DOSH immediately for death, serious bodily injury or a reportable dangerous occurrence - submit JKKP 6 within seven days under NADOPOD 2004, and

- Investigate, restore protection and verify corrective actions before authorised restart, subject to official restrictions.

Controls must be implemented, supervised and verified. Check compliance before starting work.

ONE EXCAVATION, THREE TYPES OF RISK

During drainage installation, workers entered a 2.4-metre-deep trench after overnight rain. Spoil was stacked close to the edge, a diesel pump operated nearby, and muddy water was discharged directly into a roadside drain. Cracks appeared along the trench edge, prompting the supervisor to stop work.

Applicable Standards/Regulations : (see image)
  • Safety : Rainwater weakened the exposed ground. Spoil and nearby machinery added loading, increasing the risk of collapse and burial,
  • Health: Workers faced diesel exhaust, excessive noise and contact with potentially contaminated water. Restricted access could also delay escape or rescue,
  • Environment: Uncontrolled pumping carried sediment into the drainage system, potentially causing blockage and downstream pollution.
Root cause: The excavation assessment addressed normal digging conditions but did not adequately cover rainfall, edge loading, pump location or discharge arrangements. Inspection and authorisation before re-entry were inadequate.

Simple calculation:
Assume spoil density = 1,800 kg/m³ and pile volume = 3 m³.
Mass = 1,800 × 3 = 5,400 kg (5.4 tonnes)

This illustrates substantial additional loading near the edge. It is not a trench-stability calculation; ground conditions and support require engineering assessment.

Corrective actions:
Evacuate and barricade the trench. Have a competent person assess stability and arrange suitable engineered support or sloping. Relocate spoil and machinery to assessed safe positions. Provide secure access, divert surface runoff and inspect after rain before authorising re-entry.

Position the pump away from workers, with exhaust directed away from the excavation. Assess noise and contamination exposure, and provide suitable hygiene facilities and protective equipment.

Route pumped water through appropriate sediment treatment and verify discharge quality against applicable requirements before release.

Risk Review
Using an illustrative 5 × 5 matrix, collapse risk initially scores 4 × 5 = 20/25. Residual risk must be reassessed after controls are verified, a lower score alone does not authorise entry.

Lesson: Excavation planning must address worker safety, occupational health and environmental protection together.
