HEALTHCARE - WHERE ARE WE NOW - LITTLE RESEARCH BY NIK ZAFRI

Disclaimer : I am not a qualified medical practitioner. Therefore, the following should be considered a perspective from a management consultant.



Healthcare stands as one of the pivotal elements in our existence. Over the years, the Malaysian healthcare system has showcased several positive aspects in medical advancements producing groundbreaking medical technologies, treatments, and medications that improve patient outcomes and quality of life.

There have been some improvement in terms of access especially for underserved communities, through so many initiatives beginning with the inception of telemedicine to the establishment of community health centers.

Furthermore, research and innovation are ongoing on disease prevention, diagnostics, and treatment, leading to better understanding and management of various illnesses.

I am particularly impressed with public health initiatives; especially during Covid-19 Pandemic where they actively promote public health through vaccination programs, health education, and disease prevention campaigns.

There's also a growing emphasis on multidisciplinary and collaborative care approaches, ensuring patients receive holistic treatment from various healthcare professionals.

Other notable achievements are advancements in precision medicine, where treatments are tailored to an individual's genetic makeup, leading to more effective and personalized care.

Despite the integration of artificial intelligence and big data analytics are still not been fully implemented - which is aimed to transform healthcare by improving diagnostics, predicting disease outbreaks, and enhancing treatment plans but most of my healthcare community I met - are excited considering the burdens they are facing now - having to do many things "the old fashion way" (whatever that means)

However, where there are advancements, I still notice several areas that needs improvement namely :

1) Access and Affordability

Making Malaysian healthcare more accessible and affordable for all remains a significant challenge despite efforts have been put. Addressing issues related to insurance coverage, reducing healthcare costs, and ensuring access to care for marginalized populations are crucial.

2) Healthcare Technology Integration

Enhancing interoperability among healthcare systems and electronic health records can improve coordination among providers, leading to better-informed care decisions.

3) Data Integration

Issues have arisen regarding the integration of medical data across various platforms, primarily due to privacy concerns and potential political factors. Consequently, this has led to challenges in cross-referencing and organizing patients' medical histories, potentially resulting in misdiagnosis or incorrect prescriptions.

4) Preventive Care

Shifting the focus toward preventive care and health promotion can reduce the burden of chronic diseases and improve overall population health.

5) Mental Health Support

Despite mental health education on the rise, unfortunately this issue is not being looked into seriously.

Increasing awareness and resources for mental health services is essential to address the growing mental health crisis.

6) Workforce Development

Investing in training and retaining healthcare professionals, especially in areas facing shortages, is vital to ensure adequate care provision.

Conclusion

Upgrading the healthcare system involves a multi-faceted approach, including policy changes, investment in technology, improved infrastructure, and a focus on patient-centered care. Collaboration between government, healthcare providers, insurers, and the community is key to successfully upgrade and enhance the Malaysian healthcare system.

DATUK MISBUN VS BAM



Isu BAM dan Datuk Misbun ni sudah terlalu lama dari peringkat awal sehingga kini - bagaikan retak menanti belah. Mana-mana entiti sukan apabila dikuasai politik, sudah pasti sukar bagi seorang genius sukan badminton antarabangsa ini untuk diberikan ruang menarik nafas.

Sebagai seorang genius sukan badminton negara, apabila menerima tekanan kononnya tidak mahu berkerja secara berkumpulan, Datuk Misbun bertindak berlatih secara sendirian dan akhirnya berjaya menghasilkan keputusan yang terbaik dunia.

BAM tidak pernah serik apabila Misbun menubuhkan Nusa Mahsuri dan sekali lagi berjaya mentransformasikan pemain-pemain yang tidak dikenali kepada juara-juara peringkat dunia. Mesej-mesej bisa ini masih gagal difahami oleh pihak pengurusan BAM.

Misbun adalah seorang yang cukup patriotik dan sayangkan negaranya. Kalau orang lain, mungkin dah lama lari ke luar negara dengan tawaran-tawaran yang cukup lumayan. Dan saya pasti, dia akan menerima salah satu tawaran berkenaan - malah akan ada lagi tawaran-tawaran lain akan membida Datuk Misbun. Biasalah, jika tidak dihargai di negara sendiri, sudah tentu negara lain yang akan menghargai pejuang Malaysia.

Sudah tentu Datuk Misbun berkecil hati apabila ditamatkan kontrak secara lisan. Sudah tentu dia berkecil hati apabila Datuk Subra yang menyampaikan berita buruk padanya dan bukannya rakan baiknya yang menjadi Presiden BAM. Kenapa takut berhadapan dengan Misbun? Atau ada isu lain yang disembunyikan dari kita? (memetik kenyataan Datuk Lee Chong Wei bahawa bukan semua perkara yang boleh dibincangkan di luar kerana faktor kerahsiaan - ini juga akan memberikan persepsi yang salah sebenarnya)

Bila dah kecoh Datuk Misbun buat sidang media dibantu pukulan-pukulan maut oleh adiknya, baru BAM nak kecoh. Sudah tentulah dia berkecil hati, ada antara anak-anak didiknya tidak dapat menerima caranya yang sangat tegas dan "sok-sek" di belakang beliau dan memberitahu "orang atasan". Sudah tentu dia berkecil hati apabila hanya dia seorang sahaja yang ditamatkan kontrak tidak mengikut lunas perundangan dan bukan orang lain yang ikut sama bertanggungjawab. Sudah tentulah dia berkecil hati apabila digelar "outdated" walaupun hasil yang dikeluarkannya adalah kontra dengan pendapat umum.

Saya nak tengok apa hasilnya nanti apabila dia menerima tawaran dari luar negara. Terkebil-kebil la jawabnya apabila juara-juara dunia bukan lagi datang dari Malaysia. BAM tak takut ke?

Jelas pihak pengurusan BAM tidak faham jiwa seorang genius dan mereka merasakan Misbun seolah-olah cuba "dictate" syarat kepada BAM. Walhal jika kita lihat kelab bolasepak antarabangsa mana-mana pun akan mengikut syarat yang diletakkan oleh pemain peringkat dunia.

Saya pasti persatuan badminton lain di luar negara sudah tidak sabar lagi untuk menawarkan Misbun pendapatan dan lebih lumayan tanpa berkompromi dengan syarat-syarat yang diletakkan.

Apabila kita nak dapatkan rahsia dari seorang guru, maka murid-muridnya perlulah bersabar - ini "universal value".

Saya melihat, pihak BAM bersikap tidak professional apabila mengungkit "habis duit kerana Misbun" kerana itu bukan satu kenyataan yang mendamaikan malah lebih melukakan. Tawaran BAM untuk Misbun kembali; pada saya; adalah umpama gula-gula sahaja walaupun Presidennya juga sanggup menamatkan perkhidmatannya pada hujung tahun. Kenapa perlu tunggu hujung tahun? Baiknya sekaranglah patut berhenti...baru gentleman.

Jelas, BAM tidak mahu mengakui kelemahan pengurusannya yang terpalit dengan politik dan lebih mendengar pendapat orang lain dari seorang bekas legenda negara.
Maaflah, bagi saya, kita tidak perlukan "cliche" untuk menutup kesalahan kita. Saya menyokong penuh Datuk Misbun Sidek dalam hal ini.

I NEVER HAD ENOUGH OF MALAYSIA

 Many of my foreign friends described a some sort of positive aura that Malaysia has. The peace and the unity - religion, race and culture - and still manage to integrate harmonically with modern technology/sustainable development .

You feel the joy not only in Kuala Lumpur but other states as well. I try my best to avoid highways and purposely drove via the original windy roads passing through districts and villages (you'll be surprised to find old buildings/historical landmarks along the way) to the Northern/Southern parts (and definitely east as I'm originally a Kelantan boy)
Being Malaysian, I haven't had enough of my own country, there seem to be too many interesting places that I haven't visited yet, too many hilly areas and forest reserves to explore, too many food I haven't tasted, lovely and even hidden beautiful paradise of beaches/coastal areas. (not to mention the "vast universe" of Sabah/Sarawak)
To tell you the truth, I'm still exploring (even in KL area and Selangor and trust me, I'm not only taken aback by foreign tourists admiration/respect but also surprised to see how many foreign tourists have gone to places that I've never gone before (LOL).

QUOTE FOR THE DAY - BAILOUTS AND SHARES BUYBACK ARE NOT HELPING THE PEOPLE

 When bailouts happen or corporate loans being approved with low interest rates and restructured, I think the money is intended to boost the economy by building more infrastructures and/or ease unemployment problems.

Instead; the "money" is used to buyback own shares/stocks to stabilize the price (by the "corporate borrowers"). As a result we see : two shoot ups - (1) corporate debts, (2) stock market price.
While this can being applauded, one thing remain constant, this "glory" happened while so many people including graduates are still unemployed.
Although share buybacks is legal, yet, it does not contribute to the people.
And yes, providing stimulus may be a good thing, but to those who are able to work - to have them employed is far more better.
Basics : When money is overflowing, the goods are lesser and higher in price, inflation will happen. (or is it happening?)
I hope this will not be a trend in Malaysia but I do see signs.

THINK BEFORE YOU SPEAK - (be in my shoes for a while) - EDUCATION FOR ALL in Malaysia is just another cliche!

In Malaysia, those circa 40 years and above, to access higher education, they need to be employed or sponsored by their company in order to pay for education.

My wife has been unemployed for so many years at that time but she wanted so much to upgrade her tertiary education level, unfortunately there seems to be a limit.
Alas, there is absolutely NO financial assistance given to this "category" (not even from Lembaga Zakat)
I was also working on odd jobs and I have dreamt to further my own studies, but after all consideration, I had to sacrifice my dream, used up almost all my savings, borrowing from friends not only money but also a car to send her to the place of study (and so many more embarrassing moments) - just to ensure she gets the best tertiary education for many years - finally she graduated with flying colours. ( There is a very good reason why I decided to do this )

Whilst we Malaysia are proud to proclaim using the cliche "EDUCATION FOR ALL", we keep forgetting those who wish to further their education but unfortunately neither do not have enough funds nor failure in getting any assistance from any party are not the ones the Institution of Higher Learnings wanted - be it government especially private ones.
Meaning, they are on their own! (with majority of them had to cancel their sacred intention)
So it's safe for me to say that if you're not working and don't have the money, do not DREAM to further your education.
So, stop shouting cliches that you are not able to implement. Stop comparing to other countries. Take care of this small matter first.

ISU-ISU SEMASA PASCA PEMBENTUKAN KABINET 2022 - BAHAGIAN 1 - OLEH NIK ZAFRI

ISU TELUR

Sebagai pelan jangka pendek, Kementerian Pertanian dan Keterjaminan Makanan mengenal pasti beberapa sumber dari luar untuk menampung keperluan bekalan dalam negara dengan kadar segera bagi memastikan keterjaminan telur ayam di pasaran. 

Masalah sekarang seperti yang disentuh oleh YAB PM baru-baru ini ialah menangani masalah kartel terlebih dahulu - yang telah lama menguasai pasaran dan "supply chain" - termasuk pasaraya, pembekal malah penternakan ayam itu sendiri. 

Kartel ini sangat berpengaruh - mampu "mengugut" kerajaan - bagaikan gengster, mampu menjadikan kes sorok besar-besaran malah mampu memindahkan bekalan telur (Gred C) dari stor pasaraya supaya rak-rak telur hanya meletakkan telur yang gred tinggi termasuk Omega. Ini semua berlaku kerana kurangnya ketegasan penguatkuasaan KPDNHEP.

Jika sumber luar seperti yang disebut itu berjaya dimasukkan ke pasaraya, tidak mustahil akan berlaku banyak sabotaj, contohnya kes kekurangan bekalan gula dulu, saya lihat banyak gula telah ditebuk supaya bocor di pasaraya-pasaraya yang saya lawati. 

Tidak mustahil kalau ada pihak yang memecahkan telur-telur ini atau menukarnya dengan telur yang sudah rosak kerana kawalan keselamatan di pasaraya kadang-kadang amat longgar.

Kita kena berfikir di luar kotak, jika kartel berjaya dimusnahkan monopoli mereka, saya sebagai rakyat akan rasa lebih aman kerana saya tahu kos/harga telur akan dapat dikurangkan. 

Seperkara lagi kes bekalan ayam, ini perlu diberikan perhatian serius. Kos makanan/dedak sangatlah mahal kerana kita mengimpot sesetengah bahan dari negara luar, sekurang-kurangnya ladang-ladang jagung perlu diperbanyakkan kerana jagung juga boleh menjadi sebahagian dari makanan ayam.

ISU MONOPOLI

Saya bersetuju dengan pandangan beberapa rakan di Facebook bahawa penguasaan beras oleh Tan Sri Syed Mokhtar Al-Bukhary bukan satu isu kerana harga beras masih terkawal (RM13.00 atau RM12.95) untuk beras 5kg gred rendah dan ramai lagi yang mampu membeli beras.

(walaupun atas sebab-sebab yang agak tidak munasabah, kita masih mengimpot beras dari negara jiran walaupun pesawah padi di beberapa negeri telah mampu menanam padi grednya sama. Ada yang menyebut harga beras impot negara jiran lebih murah dan kualiti tinggi tetapi masalahnya duit kita mengalir keluar. Saya lebih rela membeli dengan syarikat tempatan kerana saya tahu duit yang saya belanjakan akan menjadi pendapatan negara dan bukan milik negara jiran dan harga beras juga dapat dikurangkan) Sekiranya "monopoli" beras dipecahkan, saya bimbang kalau harga beras akan melambung tinggi sehingga harga siling contohnya RM14.50 - RM15.00 utk 5kg beras gred rendah. Sememangnya monopoli perlu ditangani, tetapi ada lagi isu-isu yang lebih penting iaitu a) Pada zaman salah seorang PM dulu, ada syarikat (kroni) yang mendapat "Central Contract" (kontrak berpusat) untuk pelbagai perkakasan ODM/OEM jentera/komputer - yang TIDAK sepatutnya berlaku. Walaupun syarikat berkenaan cuba mengadakan VDP, namun, ianya gagal kerana sistem itu menjadi terlalu mahal untuk dilaksanakan. Walau apapun konsep "Central Contract" ini MESTILAH DIHAPUSKAN. Ini juga satu bentuk MONOPOLI yang wajib Kerajaan hapuskan. (Isu ini juga pernah saya sebut pada pasca PRU-14) (tidak mahulah saya menyebut siapa pemonopoli dan juga kontraktor-kontraktor berkenaan kerana kami dalam sektor pembinaan mengenali siapa kontraktor berwibawa/berpengalaman atau kontraktor "bidang terjun" yang "membeli syarikat yang mempunyai lesen G7 serta bertaraf bumiputera dengan pengetahuan sesetengah pegawai 2 agensi tanpa melalui proses yang betul") Remember, Good Governance is extremely important and Good Governance involve TRANSPARENCY!

Lee Chong Wei, a traitor? Come on... (a quick laymen analysis by Nik Zafri)


Datuk Lee Chong Wei; as we all have known; is a Malaysian professional badminton player. As a singles player, Lee was ranked first worldwide for 199 consecutive weeks from 21 August 2008 to 14 June 2012. He is the fourth Malaysian player after Rashid Sidek, Roslin Hashim and Wong Choong Hann to achieve such a ranking (since official rankings were first kept in the 1980s), and is the only Malaysian shuttler to hold the number one ranking for more than a year.

Lee is a triple silver medalist at the Olympic Games, and the sixth Malaysian to win an Olympic medal. He won his first silver medal in 2008, also the first time a Malaysian had reached the finals in the men's singles event. This achievement earned him the honorary title "Datuk" (a national hero to Malaysians)




I'm not really here to talk about his career but I received a very disturbing "baseless rumour" on my whassap about him being accused as a "spy" and "traitor" to Malaysia?

If you carefully read this posting, you will find that the story is NOT credible as it does not provide any evidence just linking to very weak points such as :

1) LCW's cancer condition - which I think should have been respected rather than being used againt LCW 

2) Taiwan and the use of a Private Jet  - so what? He can afford it - he's here since the 80s.. 

(so what's your point? - where did he get his money from - you asked? again, do you have any other compelling evidence?)

and this posting (the only posting anyway) followed by comments containing slurs of allegations and even racism.
  • What happened to the hard copy evidences? Do you have it?

"Lee Chong Wei has revealed he once rejected an offer by a match-fixer and said he was “ashamed”  two fellow Malaysian badminton players were under investigation over claims of fixing, a report said on Wednesday."

"The 35-year-old’s revelation came after news emerged last week that two Malaysian players are being investigated by the Badminton World Federation (BWF) for suspected match-fixing."

Now I wonder, who are "these two officials" that LCW are talking about?

The pair, who are reportedly set to face a BWF hearing in Singapore later this month, could be banned for life if found guilty."

I notice there are many attempts to discredit Datuk Lee Chong Wei lately. Some even go to the extend of viralling a so-called sex-scandal video on LCW which has been debunked later. (not to mention harsh criticisms he have received in the past and when he emerged as a TV and movie icon as well)

When we think about match-fixers - we must also linked them to the "underworld" - they have people to embarass you and "kill your career" in an instant.

Please someone - make a police report before I do.

Malaysia Economic Monitor, December 2015 - Immigrant Labour







Key Findings 

The Malaysian economy continues to expand, albeit at a slower pace.
  • Growth is projected to be 4.7% in 2015, easing to 4.5% in 2016 and 2017.
  • Private consumption growth is expected to moderate from 7.0% in 2014 to 5.3% in 2015 and 2016, affected by the slowdown in disposable income and a softer labour market, though unemployment remains low.
  • Fixed investment is projected to continue expanding, driven by strong public infrastructure development, despite a downturn in the oil and gas sector
  • The current account surplus is projected to narrow from 4.3% of GDP in 2014 to 2.5% of GDP in 2015, driven by low commodity prices.
  • Malaysia has managed the downturn in commodity prices and the financial market volatility with an appropriate policy mix, notably exchange rate flexibility. 
  • The authorities have allowed the Ringgit to nominally depreciate by 20.5% from January 2015 until December 8, 2015.
  • The introduction of the Goods and Services Tax (GST) and the removal of fuel subsidies were timely and critical reforms implemented by the government. Fuel subsidy removals have led to fiscal savings of 0.9% of GDP, while GST collection is expected to amount to 2.3% of GDP in 2015.
  • These measures partly compensated for lower oil related revenues that are projected to decline from 6% of GDP in 2014 (30% of total revenue) to 3.8% of GDP (19.7% of overall revenues) in 2015.
  • Fiscal consolidation and resolving political issues can help reassure foreign investors and cope with heightened external vulnerability.




Immigrant labour plays a crucial role in Malaysia’s development. Immigrants – both high- and low-skilled – will be needed for the country to achieve high income status by 2020.

  • Malaysia has the fourth largest number of migrants and the seventh highest ratio of migrants to total population in East Asia Pacific.
  • There are 2.1 million registered immigrants in Malaysia and likely over 1 million undocumented immigrants, making up 15% of Malaysia’s workforce in 2014.
  • As Malaysians have become more educated and seek out higher-skilled jobs, and as the labour market remains tight, immigrant labour has filled gaps in low- and mid-skilled jobs, which make up three quarters of all jobs in Malaysia.
  • Econometric modeling suggests that a 10% net increase in low-skilled foreign workers may increase Malaysia’s GDP by as much as 1.1%.
  • For every 10 new immigrant workers in a given state and sector, up to five new jobs may be created for Malaysians in that state and sector, two of them female.  
  • Economic modelling suggests that a 10 percent net increase in low-skilled foreign workers may increase real GDP by up to 1.1 percent. 
  • A 10% increase in immigration flow slightly increases the wages of Malaysians by 0.14%. Yet it significantly reduces salaries of immigrant workers already in the country by 3.94%.  A 10% increase in immigration flow reduces wages of the least-educated Malaysians, which represent 14% of the total labour force, by 0.74%.
  • The fiscal burden is mainly borne by undocumented immigrants. Documented immigrants in Malaysia raise employment and wages of Malaysians which in turn contributes to public revenues. Also, levies pay for work permits of documented immigrant workers raising 1.2% of total revenues in 2014, they have to have health insurance which reduces the burden on the government.




Recommendations: Six possible directions for reform for Malaysia to strengthen its immigration system.

  • Aligning the institutional and legislative framework with the human resource development strategy;
  • Establishing an evidence-based system for identifying labour market shortages that immigrant labour can fill;
  • Adopting a live-levy system that responds to labour market needs identified in the evidence-based system;
  • Using a broader set of criteria to categorize immigrants (i.e. skills, experience) and defining approaches for their recruitment, employment, and repatriation;
  • Strengthening monitoring and enforcement of immigration and labour regulations; and
  • Investing in upskilling the unskilled workforce, and promoting productivity-enhancing technology.

AIN'T WE ALL FROM AN ALLIED NATION? BUKANKAH KITA INI SERUMPUN?

A long time ago, I was in Indonesia to accept an invitation from an Indonesian business partner. My visit was for the second time (where 2 years before I was there)

Despite of whatever people were saying that 'things haven’t changed much', I still have the confidence that there have been changes.

Thus, I got into a 'little trouble' with the authorities due to my lack of understanding how to 'settle problems' despite I’ve been there before.

Out of nowhere, a group of good Indonesian Samaritans emerged to the 'rescue'. I could not have done anything without their help. I gave them some money in appreciation to their assistance but they denied it by saying:

 "Kami tolong bukan kerana uang Pak - kita ini serumpun" 
 (We're not helping because of money Sir, it's because we are of the allied nation (from the same root)"

I was touched...Not all are Malays or Muslims - they are all of diverse-races and religions.

In the early 90s, I was hired as HRM for a national project, I noticed Indonesian foreign workers (skilled) were having 'problems' and 'limitations' due to strict conditions imposed by the *sub-contractor hiring them. *(labour-supply supplier/sub-contractor)

Their passports being withheld by respective agencies or the sub-contractor (fear of the foreign workers might run away and I don't blame the contractors), the levy (has to be paid by the workers - monthly deducted from their wages) and income tax rebates/EPF/Socso (if any and wherever applicable) belongs to the employer once they decide to return home (with exceptional to nett-salary - which they get to keep (minimal amount) and the travel documents will be returned to them) - and some 'people' were imposing 'extra charges' per head per month. But what touched my heart most was 'their living conditions" - the so-called 'labour quarters' and some 'inhumane' treatments (which I rather not say)

After serious discussions with the Client and the Main JV Contractors, we decided to take over the foreign workers to be directly under our payroll and the employers (majority : Japanese) have unanimously agreed that I would be in charge of the Indonesians foreign workers welfare. We have paid a hefty sum to compensate the losses of the previous employer. From there things started to change. But there were still old 'habits' where the Indonesians thought that I am the 'new 'tyco' (local colloquial - nearest translation - like some sort of a godfather - LOL) and that they have to pay 'a certain amount of fees' - I denied their gifts of course which have left them baffled. (personal note : Not all Indonesians workers are Muslims, there are Christians and Buddhists as well)

Some still asked "You mean Mr Nik, that all this money we get to keep?". I chuckled and told them 'YES'.

They were still in awe when they get 'hefty' annual bonus (few months) and the levy were borned by the employer and not them, there were no more 'unscrupulous' people to charge them 'per head taxes', no more gangsters coming after them, their past loans were paid fully, their family have moved to better labour quarters - not a single cent need to be paid for rent etc. etc.

In short, they were FREE. (they get to keep their travel documents, passports (of course - an agreement needed to be signed first, they were never left behind in trainings - in fact, they received certificates of trainings)


So to cut the the story short, when the project was about to be completed, we gave them a handsome compensation, I personally handled their income tax rebates - all returned to them, immigration matters, they get to keep their final salary, I (on behalf of the company) even bought the air-tickets for them.

I got to meet almost all of them for the last time in a small farewell gathering together with their families before they left - returning to Indonesia. They all asked me the same questions :

"Abang Nik, kenapa Abang Nik begitu baik dengan kami? Kami tidak beri apa-apa wang pun" (Bro Nik, why are you so kind to us? We didn't give you any money.)

I replied : 

 "Saya tolong bukan kerana uang - bukankah kita ini serumpun?" 
 (I am not helping because of money, ain't we all from an 'allied nation' (from the same root)"

They were shocked and there was a moment of 'suspended animation' (silence)

Then I told them my little story - it was a teary drama and everyone prayed for my good health and prosperity.


The moral of the story is :

"It's easy to turn a blind eye on people we do not know - but be nice to visitors - trust me, somehow you will get the same treatment when your turn comes in and you have to be a visitor to them"
